Practice Policies

  1. Registering with the Practice
    We are pleased to register any new patients who live within our catchment area. To register as a patient, please complete our Consideration for Registration form which is available from Reception. On completion please return it to Reception and it will be discussed by the Doctors at their weekly Practice meeting
     
  2. Your Personal Information
    In order that we may contact you, please ensure that we have your current telephone number and address, and also contact details of a friend or relative in case of an emergency. If you have a new or an updated medical card, please present it to the receptionist so that it can be recorded in your file
     
  3. Completion of forms
    Requests for completion of all forms, including applications for medical cards will take a minimum of 3 working days. All forms left in for completion must be signed by the applicant and all necessary sections completed. Some of these forms are not covered by the Medical Card scheme and will incur a fee.
      
  4. Patient responsibility
    In the interest of health and safety, and out of consideration for other patients, it is essential that children are encouraged to behave. All young children must be accompanied by an adult when attending for consultation with either a doctor or nurse.

    Patients who repeatedly fail to attend for appointments, or are physically of verbally abusive to our staff, doctors or in front of other patients, may be removed from our list.
     
  5. Teaching Practice
    The practice has for many years been involved in the education of medical students. At periods throughout the year students will visit the practice as part of their training. Patients will be informed that a student is present before their consultation with the doctor and their consent obtained.
     
  6. Confidentiality
    Your medical records are held in the strictest confidence. The practice is computerised and registered under the Data Protection Act.

    We constantly review the way the Practice operates and would therefore welcome any suggestions for improvement. Any complaints should be put in writing and addressed to our Practice Manager who will ensure they receive our full attention.

    Full Health & Safety and Fire Policies are in place and updated regularly.
